Shuttle Request Form

Departments requesting the use of a University shuttle and driver must complete this form in full. The requesting department will be charged for the cost of the driver and fuel. 

Please note that requests are not guaranteed and are based on driver and shuttle availability. Each shuttle has a capacity of 14 passengers + the driver.

Shuttle Rate is $200 per shuttle up to 4 hours and $50 per additional hour. This cost includes fuel.
