In response to increased rates of COVID-19 transmission in other countries and throughout the United States, and to comply with federal, state and local quarantine mandates, all faculty, staff and students who have recently traveled, or who are planning to travel outside of the state of New York, must complete this form. This applies to all international travel as well as out-of-state domestic travel, unless such travel is to a state that is contiguous to New York (i.e., New Jersey, Pennsylvania, Connecticut, Massachusetts, and Vermont). Individuals traveling to a contiguous state do NOT need to complete this form and do NOT need to comply with New York's COVID-19 Travel Advisory.
Individuals who travel outside of New York and its contiguous states will need to comply with certain quarantine/testing requirements as promulgated by New York State. Molloy's Student Health Services Office will contact travelers and inform them of these requirements. All submissions are private and will only be shared with those who need to know in the event of an emergency.
