In response to increased rates of COVID-19 transmission in other countries and in certain states within the United States, and to comply with federal, state and local quarantine mandates, the College must know where all members of its community have traveled or are planning to travel. Therefore we request that all faculty, staff and students who have traveled outside of the state of New York in the previous fourteen (14) days, or who are planning to travel in the foreseeable future, fill out this form. This information is private and will only be shared with those who need to know in the event of an emergency. The Student Health Services Office will contact any individual who is required to quarantine.
