Chosen, Preferred, or Lived Name Request Form

A Chosen, Preferred, or Lived (CPL) Name is an important part of a person’s identity. The University recognizes that students and employees may want to use a CPL Name that is different from their Legal Name.

Please use the CPL Request Form to update your CLP name. Once you submit your request, it will be routed to Student Affairs (for students) or Human Resources (for employees). Once processed, please allow 48 hours for your CPL name to reflect in all applicable university systems accurately.

Pronouns and Gender Identity


Students and employees can update their pronouns on their own using Canvas and employees can update their pronouns on their own using ADP Workforce Now and Microsoft Webmail. Using your pronouns in email signatures and other profiles (e.g., Zoom) is also a good practice.

Note: It should be understood that even when pronouns have been entered into University information systems, an individual’s legal sex will still be used by the University whenever required by law or a University business need.
